Purpose of form: This form provides employers, accountants, bookkeepers and software companies the specifications for preparing a computer generated Wages List. This form is used when more than 5 employees need to be reported on the Employer's Quarterly Report (Form C-3). See Computer Specifications for Form C-3 Employer's Quarterly Report (Form C-3SP) for details.
Who can use this form: This form can be used by any individual or organization wanting to use their computer to complete the Wages List.
Requirements: The printer specifications require that data is to be printed using black
ink at 10 characters per inch, 6 lines per inch, double spaced. The paper specifications require
that if paper is to be other than TWC provided forms, it must be white, 20 lb, 8 1/2 X 11, OCR
grade - void of horizontal or vertical lines. The "shade" of green for the form is PMS354
with 20 percent shading.
Anchor Points:
Each leg of anchor bar is 1/4" in length. Thickness of bar is minimum 0.03" and maximum 0.05".
