Employers or their representatives sometimes request that a special address be established for the
mailing of Form C-66, Notice of Maximum Potential Chargeback. This form advises employers of possible
charges to their account in the event that a claimant receives unemployment benefits. Special addresses
are exclusively maintained by the UISS - Room 354. Always send any special address
information to that department. If possible, email information using the global address "UI
Central".
Current status of special addresses can be obtained by keying SPS on the command line (CMD). Only one
special address will be available on this screen. If an address appears on the SPS screen, then that
address can either be changed or deleted by UISS. If one does not appear, then a special address can
be added.
All forms including Form C-66 will be mailed to the address of record when no special address is on file.
Special address requests are usually included along with Written Authorization (C-42), but can
also be in regular correspondence. In any event, when an accounts examiner receives such a request in
writing the accounts examiner will process any tax actions first and then:
Designated and chargeback address requests are also handled in the same manner. Designated (claims)
Address requests allow the initial claims information to be sent to the designated address instead
of the address where the employee last worked. You can view this address, if applicable, by keying
CMDA (Designated Employer Address Change) on the CMD line.
