6.3     C-3s Reflecting No Liability

[ 6.3.1 - Investigations Resulting in No Liability ]

Investigation is needed for Quarterly Reports received showing less than $1500.00 with no money and with no Status Report. See Chapter 1 – “New Accounts 201.024” for reports less than $1500.00 with money.  The following outlines the procedure to be followed in this type of investigation:

  1. Send FL-164 to request a Status Report and trace for 60 days.

  2. Make comments on the FTC screen giving pertinent information.

  3. After the trace is returned at the end of the sixty day period, check the Transaction Log (TDO) to see if the documents have been received.  If there are documents received, see Chapter 1 - "New Accounts 201.021".

  4. For documents not received, initiate a field assignment for in-state employer accounts or send FL-164 Second Request for out-of-state employers requesting a Status Report (trace for 60 days). Document FTC.

6.3.1     Investigations Resulting in No Liability

If there is no response to FL-164 Second Request from out-of-state employer:

  1. Change the account to "not liable" using the STA screen.

  2. Send e-mail to ADP & Audit to delete reports and document FTC.
