Employer Registration & Account Update

Use the forms below to register or update your unemployment tax account:

  • Employer's Registration - Status Report - (Form C-1) enables TWC to establish a new account for a non-farm employer. Print this form and mail it in or register online.
  • Farm & Ranch Employment Registration - Status Report - (Form C-1FR) enables TWC to establish a new account for a Farm or Ranch employer. Print this form and mail it in or register online.
  • Amended Status Report - (Form C-1AM) notifies TWC of a change in the status of an employer account. The changes can include a change in officer or owner or the acquisition or sale of a business. The form is for use by employers with existing TWC tax accounts.
  • Status Change Form - (Form C3SCF) Status Change Form - (Form C3SCF) MS Word or Status Change Form - (Form C3SCF) PDF notifies TWC that a business has been discontinued, business is continuing without employment or the business has been acquired by a successor. The form can also be used to correct a name, tax mailing address, account number, Federal ID number or a telephone number that has been omitted or is incorrect on preprinted tax forms.
    Note: To change a designated address for receiving notices of unemployment insurance claims filed and determinations made on those claims, see Designate an Employer Mailing Address.
  • Notice that Employment/Business Discontinued - (Form C-13) notifies TWC that a business has been discontinued or no longer has employees.
  • Transfer of Compensation Experience Questionnaire - (Form C-38) helps TWC determine if a tax rate issued to the previous owner can be transferred.

Tax Report Filing & Payment

TWC Rules 815.107 and 815.109 require all employers to report Unemployment Insurance (UI) wages and to pay their quarterly UI taxes electronically. Employers that do not file and pay electronically may be subject to penalties as prescribed in Sections 213.023 and 213.024 of the Texas Unemployment Compensation Act (TUCA)

Employer's Quarterly Wage Report Filing Options – To use Internet Wage Report Filing or QuickFile, or submit a Domestic Employer's Annual Report you must be a registered user of Unemployment Tax Services. Register at Unemployment Tax Services. Quarterly wage reports can also be submitted through Intuit or by magnetic media. Below are forms you can use to file quarterly wage reports or submit tax payment if you have been granted a hardship waiver by the TWC:

  • Domestic Employer's Annual Report - (Form C-3DOM) and Continuation Sheet (Form C-4DOM) allows domestic-only employers who have elected to file annually to report their employee wages. Print this form and mail it in or file it online.
  • Payment Voucher - (Form C-3V) allows employers or their representatives to submit their personal check payments to the Texas Workforce Commission. This form is intended to assist employers when their representative files their tax report using electronic media and the employer wants to pay the taxes due with a personal check.

Adjustment Reports

Use the forms below to adjust previously submitted wage reports:

  • Adjustment Report - (Form C-5) corrects amounts of taxable and/or total wages previously reported on the Employer's Quarterly Report, Form C-3. Print this form and mail it in or complete it online.
  • Wage List Adjustment Schedule - (Form C-7) corrects total wages previously filed on the Continuation Sheet, Form C-4. If the Social Security number, name or wages of one or more employees were omitted or erroneously reported, correct each error on this form. Print this form and mail it in or complete it online.
